Thank you for being a valued Food Truck Partner. This Page can be used to upload requested documents to AAFT. Although you may have submitted documents to us in the past, many licenses and certificates expire. Please use the corresponding forms below to upload requested documents.
Insurance Form:
C.O.I. / Certificate of Insurance
A certificate of insurance is a document that provides proof of an insurance policy's existence and summarizes its key details and coverage limits.

When requesting a COI from your insurance provider, be sure to have "All About Food Trucks" listed as an additionally insured.

If AAFT has asked you to have a COI issued for a specific client, be sure to provide your insurance provider with the exact details that have been requested.

MFDV License:
DBPR / Dept. of Agriculture
Your MFDV license legalizes you to do business as a Mobile Food Dispensing Vehicle (MFDV). Please Note: Regulating Agencies may differ by Region, However, you must be licensed to work as a Food Truck with AAFT.
Food Manager Certification:
Safe Food Handling Compliance
Food Managers are required to obtain regulatory approved Food Manager training and certification. Requirements may differ by region / jurisdiction. In order to work with AAFT, a Food Manager Certified member of your team must be present during your scheduled shifts and we must have a valid certificate on file.
